The CHANGE Program has developed as a grassroots initiative, with leadership from healthcare, academic, government and community partners. This collaboration is making the CHANGE Program available to more and more Canadians. The leadership of the CHANGE BC team is a great example of communities working together:

Metabolic Syndrome Canada – seed and ongoing collaborative partnership funding
BC Ministry of Health – Primary Care Network Funding – Permanent funding as part of the Coast Mountain Primary Care Network and Bulkley Valley Witset Primary Care Network, with more PCNs moving forward with developing local CHANGE programing.
University of British Columbia School of Kinesiology – students, curriculum, and evidence based patient teaching material, and training supports, including a strong focus on cultural safety
University of British Columbia Faculty of Land and Food Systems – students training supports, and evidence-based patient teaching material, including a strong focus on cultural safety
