Selecting a Program
In selecting a program to help patients living with MetS and Type 2 Diabetes, the Pacific Northwest Division of Family Practice, together with Metabolic Syndrome Canada, University of British Columbia’s School of Kinesiology, Faculty of Land and Food Systems, and Faculty of Dentistry, local First Nations Health Directors, Nisga’a Valley Health Authority, Northern Health, Doctors of BC, BC’s Ministry of Health, and other collaborative partners have moved forward with CHANGE BC to support:
evidence based patient care that drives health system improvement within Primary Care Networks;
Nation driven and community led supports;
individualized, wellness focused patient care, offered in a manner that places the highest priority on cultural safety, removes barriers for patients and is responsive to feedback;
a collaborative team-based approach that prioritizes Family Physician-Patient relationships and team continuity, where the patient sees the same provider and team, generating better patient outcomes;
communities of practice at a local, provincial, and national level to share resources and tools to support primary care transformation, improvement, innovation, and excellence through the CHANGE model.
